Police Academy
Application Process
Florida Community
College at Jacksonville (FCCJ) and Northeast Florida Criminal Justice Training
and Education Center (NEFCJTEC) require that applicants for all state approved
law enforcement training programs complete a strenuous application process prior
to their admission in any basic training course or program.
You Do
Not have to be a college
graduate to attend this course.
Application
Process Phases
The physical fitness test is administered
every third Saturday (except in December) at the Police Academy @ 9:00am. Be prepared
to perform sit-up, push-up's a 1.5 mile run and an officer mobility course. Failure
to successfully pass the PT test eliminates you from receiving an orientation
and application to the Academy. Applicants may retake the exam each month for
consideration until successful.
Once you pass
the PT Test you will receive an application packet. This packet provides directions
on what must be completed and submitted. Follow the instructions! Failure to complete
the initial application or providing false information will eliminate you from
further consideration.
Upon successful completion of the first two phases, you will
be scheduled to attend an oral interview. The interview will last at least one
hour however you should be prepared for two. Failure to pass the oral interview
will eliminate you from further consideration.
After you have successfully completed the first three phases, you will be notified
that you have been selected for the Academy program. You will receive specific
instructions on the class starting date, time, location, uniforms, etc. in that
letter. You will receive a current course cost sheet for the payments, dates,
uniforms and other information when you receive registration directions.
The
Following Documents Will Be Required From Potential Students:
- Must
be at least 19 years of age
- Verification
may be accomplished by providing a copy of your birth certificate
Baptismal certificate is NOT an acceptable verification of age
- Copy
of birth certificate
- Copy
of court documentation attesting to citizenship in the absence of a record
- Current,
valid U.S. passport
- Report
of birth abroad of a citizen of United States( issued by a consular office)
- Current,
valid passport
- Department
of Immigration and Naturalization Form G-641
- Certificate
of Naturalization from the Department of Immigration and Naturalization
- Naturalized
Citizen - Certificate of Naturalization
- Sealed
Transcript from your High School
- Out-of-state
GED, with transcript of test results. Applicant must have achieved an aggregate
score of 225 on all five sections of the test, with no score below 40 on any single
section. If any test section has been scored below 40, the applicant must retake
that portion of the Florida GED.
- Post-secondary
degree issued by an accredited institution
- Official
transcript issued by an accredited post-secondary institution identifying an
associate degree of successful completion of at least one year of college
work. - High
school equivalency diploma (USAFI) certificate issued prior to December 31, 1974,
by the United States Armed Forces Institute denoting successful completion of
high school equivalency examination.
- Official
(raised seal not photocopy) college transcripts.
- If
you possess an Associate or Bachelors degree submit photocopies of the degree
and copies of your college transcripts.
- Statement/Certification
of Medical condition.
- You must have
a basic physical examination to participate in this program.

This physical cannot be more than one year old as of the
start date of the class.
- You are required to obtain
an examination documented on the enclosed FDLE/CJSTC Forms 75 & 75A.
- The
FDLE/CJSTC Form 75A must be completed by a licensed Florida physician of your
choice.
- If you are in the military any DOD physician
authorized to treat DOD patents may complete this examination.
The
Academy uses the Jacksonville Sheriff Office, Drug History Disqualifiers list
as its guiding rule in evaluating and admitting all potential applicants to recruit
training programs.
- You
must submit a copy of your Florida driving history.
- Take
the enclosed form to the Clerk of the Circuit Court, 1478 Park Ave. (Hwy 17),
Park Central Plaza, Orange Park, Florida. There is a fee for the Driving History
check.
- Call 904.278.4769 to ask about the
fee.
- The office is open Monday thru Friday
9 a.m. - 4:30 p.m. and Saturday 9 a.m. - 1 p.m.
The

- A
photocopy of your current valid state driver's license (front & back) is required.
- Please
enlarge the print size to double the normal license size when making this copy.
- A photocopy
of your social security card is required.
- If
you do not have your card it takes 4-5 weeks to obtain a new card.
- You
will receive an affidavit that you will need to sign, date and return.

Both Applicant and a Notary Public must sign the affidavit
form stating your criminal history and military history.
- You
must maintain current health/accident insurance during the training course.
- Copy
front and back of your insurance card is required. (Please enlarge the printing
size.)
- If you do not currently have health/accident
insurance you must obtain coverage before the first day of class.
- Active
duty military personnel may provide a copy of your military ID card as proof of
insurance.
- The college does have a program
to provide insurance to cover you during class hours.
- You
must register for this course approximately three weeks prior to the course start
date.
- A Vocational Course Assignment sheet
will be provided by mail.
- You must pay
approximately $500 of the course fee when you register.
- You
will also complete a FCCJ Application for Student Membership.
- The
FCCJ system will permit you to register at any FCCJ Campus or on line.
- It
is HIGHLY recommended you register in person at the North Campus as they are familiar
with the Academy programs.
- Florida Resident
Information is required.
- Provide a copy
of your receipt indicating registration and payment for Module I on the first
day of class.
- Your FBAT Score
Sheet must be submitted.
- FBAT Cannot be more than
two years old as of the class start date.
- Contact
campus testing centers to obtain information on the FBAT Test.
- A photocopy of
your DD-214s.
- You
will be provided with a list of polygraph examiners to obtain a polygraph examination.
- The results will be provided to the academy.
